Rehabilitation service delivery systems

Rehabilitation can occur at the acute level hospital, the non acute inpatient setting or in the community. Below is an overview.

Hospital Level

This level is for patients with an acute medical need that requires ongoing medical care.

  • Acute care hospital
  • Long Term Acute Care Hospital (LTACH)

Other Inpatient

Inpatient Rehabilitation-provides intensive rehabilitation a minimum of 3 hours a day, usually by multiple disciplines.

SNF-Provides rehabilitation services at a less intense level and for less than 3 hours a day for patients who cannot tolerate Inpatient Rehabilitation

Community Rehabilitation

Day Program-for patients who no longer require hospitalization but still need intensive, coordinated rehabilitation.

Outpatient Rehabilitation- Patients travel to a clinic or hospital to attend sessions and then return home the same day. Typically, a therapy session lasts from 30 minutes to an hour.


Home Health-Therapy is provided in the patients home for up to an hour per discipline per visit. Visits are usually made up to 3 times per week.
