By definition the Interdisciplinary Care Team (ICT) is a team of healthcare professionals from different professional disciplines who work together to manage the physical, psychological and spiritual needs of the patient. Whenever possible the patient and the patient’s family should be part of the team.
Members of the Interdisciplinary Care Team may include:
- Physicians
- Nurses
- Case Manager
- Social Worker
- Physical Therapist
- Occupational Therapist
- Chaplain
- Dietitian
- Pharmacist
It is the hospital Case Managers responsibility to assess the patients post hospital needs and resources available. Once the needs are identified, the Case Manager will facilitate referrals to appropriate community based provider and services.
